This is FASCO 7126-3040 Blower Motor Heil ICP HQ1008415FA 115V 1/3 HP RPM 1075 3 SPD used part.
This part was removed by HVAC installer from a working unit.Old system was replaced with newer one (more energyefficient) and was in good working condition.
